Cancellation Policy

If you are unable to meet at your scheduled therapy time, please contact me at least 24 hours before the appointment. Missed appointments or appointments canceled less than 24 hours in advance (barring emergency or illness) will result in a late cancellation/no-show fee that is the full cost of the appointment*. Please be aware that missed appointments cannot be charged to insurance & the fee will be the responsibility of the client*. More details relevant to timely attendance and communication are included in the intake paperwork packet for your review.

*Clients with Medicaid will not be billed.

Client Portal

If after your initial consultation we both decide to proceed with scheduling an intake assessment, I will send you an email with information about creating an account on the client portal CarePatron. Here you will be able to view and sign the required intake paperwork. You will be asked to complete an additional form or forms asking about your information, medical & mental health history, & current symptoms. For clients under 18 of age, these forms must be signed by a legal guardian. If a minor has more than one legal guardian, all legal guardians must be informed of and consent to their child's treatment. If you are using your private health insurance, you will be sent a link to create an account on Headway, which is an insurance billing platform, where your insurance will be verified and where you will submit payments. If I do not take your insurance or if for any reason you choose not to use your insurance, payments can be submitted via the CarePatron portal and I will provide you with a Superbill upon request which you can submit to your insurance company to count towards your out of network deductible.
